Diabetes Transition Service

This service has been especially designed to meet the needs of young people aged between 16-19 years whilst preparing to transition into adulthood, from paediatric to adult diabetes services.

Transition clinics are held at:

West Cumberland Hospital (WCH) – Outpatients area 2 with Dr Holt Davis & Dr Idampitiya

Cumberland Infirmary (CIC) – Children’s outpatients with Dr Sikkander & Dr Fatima

During these appointments you will be seen by both the paediatric and adult diabetes teams. Referral to dieticians and psychology can be made if and when needed.

Transition service

Responsibility for your diabetes care will remain with your paediatric diabetes team until you transfer to the young adult service. The transition team will help you to decide when this is the right time for you (between 18-19years).

You will be encouraged to start becoming independent in managing your diabetes and will begin to develop relationships with the adult diabetes team.

Clinic appointments

You will continue to be offered 4 appointments per year when in the transition service. In these appointments you will continue to have your annual review which will include:

•            Height, weight & Blood pressure

•            Urine sample (To check your kidney function)

•            Blood test 

•            Foot screening.

Please continue to attend your retinal screening appointments.

When you first attend the transition clinic, you’re very welcome to bring someone with you, such as a parent or carer. As you become more comfortable with the clinic, we would encourage you to start attending appointments on your own, without your parent or carer.

When you turn 16 years old, you will need to show an exemption (from payment) certificate when collecting your medications. Please see this link for further information:
