What the team does
- Work with stroke survivors, their family, carers and the hospital stroke team to ensure an early discharge from hospital.
- Provide specialist stroke rehabilitation in the home and community environments. This may be through face to face visits, telephone or virtual appointments. It may be a combination of these.
- Provide specialist advice and education on stroke recovery and health promotion to survivors, carers and families.
- Work with the stroke survivor to provide personalised rehabilitation to support those affected by stroke to make the best possible recovery.
- The team works with those affected by stroke to empower them to self manage their long-term condition
How will it benefit me?
- Provide a smooth process between your rehabilitation on the ward and rehabilitation at home.
- Improve your ability to carry out activities important to you.
- Help you learn more about your condition.
- Improve your quality of life and support your family, carers or partner.
What is required of me?
- You will be expected to take part in your rehabilitation programme and practice rehab tasks set by your therapists.
- As part of the therapy programme the team will help you to identify therapy goals to work towards as well as advice on self-management.
- There is no set time as to how long this will take. It will be dependent on your needs.
Rehabilitation and goals
- With the team, you will set goals that you want to work on as part of your rehabilitation. You will practice tasks related to these.
- The team will work closely with you so that you understand and practice what you need to do for the best possible recovery.
Planning for discharge from the team
- The team will work with other services to make sure support and rehab continues where needed.
- The team will work with you and carers to ensure you have the resources to be independent in managing your condition and long-term goals.
What happens when I finish with the team?
- By the end of your rehabilitation with the team you will have a better understanding of how to manage your condition and longer-term recovery. The team will have gone through with you how to do this and what areas of rehabilitation you can continue with.
- There is local support available for people who have suffered a stroke. Details of these are in your information pack.
