The Palliative Care service is for people who need more care than can be provided by family or carers. Some of the people we help:
- are generally frail and have other complex conditions
- have a life-threatening condition caused by a sudden event such as an accident or stroke
- have an incurable illness such as cancer or dementia
Our team is made up of specialist nurses, doctors and allied health professionals such as:
- clinical nurse specialists
- consultants and speciality doctors
- specialist occupational therapists
When you’re referred to us, we’ll work in partnership with your own GP, nurses, social care and local charities to care for you. We’ll assist with but do not take over your care.
Some of the things we may help you with are:
- assessing and managing difficult symptoms
- emotional, psychological and spiritual support
- advice on social, financial and practical issues
- information about your treatment and care
- practical help with useful equipment
- support for family and carers
- information about other agencies that can help you
